License Application

 

When applying for the following licenses you may obtain an application from the local Registrar’s Office, the Borough Website, or the link under "Helpful Forms".

 

  • MARRIAGE LICENSE

  • REMARRIAGE
  • DOMESTIC PARTNERSHIP

  • CIVIL UNION

  • REAFFIRMATION OF CIVIL UNION

 

 **Do not sign the application form before going to the Local Registrar. You must sign the application, while under oath, in the presence of the issuing authority.**


 

Certified Copy of Vital Record

 
All requests for birth, death, or marriage certificates must be either mailed to the Borough of Mountainside, 1385 Route 22 Mountainside, NJ 07092 or in person.

 

  • IN PERSON:
    Original documents must be presented. Payment must be in the form of a check made out to the Borough of Mountainside or cash in the amount of $15.00 per copy.
  • BY MAIL:
    Photocopies are acceptable. Payment must be in the form of a check made out to the Borough of Mountainside in the amount of $15.00 per copy.

People who are homeless can provide identification from a social worker or homeless shelter coordinator. People who are incarcerated can provide legal imprisonment, conviction papers, or release documents that include the name, social security number, and all possible aliases use in the past or identification from a prison/probation official.
