Click on the links below to download registration forms for select programs. Although forms may be printed in advance, they will not be accepted by the Recreation Department until the stated registration period begins. Forms may be returned in person or by mail, with payment included (checks, cash, and money orders only – no credit cards). Forms received after the stated registration period must include a $15 late fee. Sports programs are first-come, first-served. Enrollment limits are based on available volunteer coaches. A $10 administrative fee will be charged when issuing a refund for a recreation program or activity. Call (908) 232-0015 for more information.
